NEW PATIENT GUIDANCE
Once you have scheduled your appointment with us, please follow the steps provided below.
1. Download, print, and fill out the new patient intake form. Please make sure they are completed and turned into our office manager the day of and/or before your appointment. If you have any questions regarding any of the forms, please feel free to call our office. Also, please bring your insurance cards in case of lab draws. If you are not able to print the forms, or cannot complete them beforehand, please arrive to your appointment at least 15 minutes early.

2. Be on time for your appointment. If you need to reschedule or cancel your appointment for any reason, please call the office as soon as possible or within 24 hours of your appointment. Our goal is to provide quality individualized medical care in a timely manner. "No-shows" and late cancellations inconvenience those individuals who need access to our care in a timely manner. We would like to remind you of our office policy regarding missed appointments, a $50 fee, if you don't show up to your appointment or cancel within 24 hours; this policy enables us to better utilize available appointments for our patients in need of medical care.
